Community Connect is a North Dakota state-sponsored, community based behavioral health program designed to assist individuals living with a mental health or substance use disorder find their own recovery.

Step1-Graphic

CARE COORDINATION

This is an ongoing source of connection with a Care Coordinator. A care coordinator assists with short and long-term goals, and barriers, and provides a source of connection and support.

Step2-Graphic

RECOVERY SERVICES

Includes access to services or programs to assist an individual with meeting their needs and goals.

Step3-Graphic

PEER SUPPORT

Connection with a peer who has similar lived experience. Peer support specialists provide mentorship, advocacy and support.

Building connections for your recovery

TO BE ELIGIBLE FOR COMMUNITY CONNECT, INDIVIDUALS MUST:

  • Be a North Dakota Resident
  • Be  of the Age 18+
  • Have a Mental Health or Substance Use Disorder diagnosis causing challenges in areas such as housing, employment, financial, parenting, physical health, and community connections
