CARE COORDINATION

This is an ongoing source of connection with a Care Coordinator. They’ll help participants access treatment and recovery support services, and creatively addressing barriers to individual success. It also includes the provision of assessment, care planning, referrals, and monitoring collaboration with clinical services and probation and parole if needed.

Step2-Graphic

PEER SUPPORT SERVICES

Connection with a peer who has similar lived experience. Peer support specialists provide mentorship, advocacy and additional recovery support.

HOUSING SUPPORT

Housing support is available to help individuals’ access and maintain stable housing in the community. Services are adaptable and personalized as they involve teamwork between service providers, property managers, and participants to ensure housing engagement, maintain tenancy, and handle any crises that might come up.

Requirements

A care coordinator develops a person-centered plan of care and assists in gaining access to needed 1915i services.

TO BE ELIGIBLE FOR 1915(i), INDIVIDUALS MUST:

  • Age 0+; and
  • Enrolled in North Dakota Medicaid or Medicaid Expansion; and
  • Have a household income at or below 150% of Federal Poverty Level; and
  • Receive a WHODAS score of 25 or above; and
  • Reside in and will receive services in a setting meeting the federal home and community-based setting requirements; and
  • Have a diagnosis of substance use disorder, mental illness or brain injury
